True North Recovery Services (TNRS) is committed to protecting your health information. We are required by law to maintain the privacy of your protected health information (PHI), provide you with this Notice, and abide by the terms described here.

Treatment: To provide, coordinate, or manage your healthcare. For example, we may share information with hospitals or other providers if you become ill, require hospitalization, or need follow-up care.

Access: You may request to see and receive a copy of your health records.
Amendment: You may request changes to your records if you believe they are inaccurate or incomplete.
Restrictions: You may request limits on how we use or disclose your information.
Confidential Communications: You may request to be contacted at a specific address or phone number.
Accounting of Disclosures: You may request a list of disclosures we have made of your PHI.
Paper Copy: You may request a paper copy of this Notice at any time, even if you have agreed to receive it electronically.
